And in the case of AI, the stakes are real: avoiding biases, ensuring transparency, maintaining a central role for humans.<h4>Secondary data: should barriers be lifted?</h4>Ségolène Aymé (INSERM), Léa Rizzuto (Health Data Hub), and Frédéric Lesaulnier (Brain Institute) opened the debate on a sensitive subject: the use of secondary health data. While their potential is enormous for research, their exploitation remains hindered by regulations sometimes considered too rigid, especially in France.Yet, all agree: the benefit-risk balance favors their use, provided that the protection of primary data is impeccable. She reminded that risk management is at the heart of recent texts such as the European AI Act, with a structured approach aimed at anticipating risks related to discrimination, privacy, and data manipulation. Special vigilance is required across the entire AI system value chain.<img src="https://f4c-strapi.s3.gra.io.cloud.ovh.net/article_ethique_2_e729ee9b1a.PNG" alt="article éthique 2.PNG" width="751" height="411"><h4>A trusted AI? Yes, but under what conditions?</h4>Acceptability, explainability, interoperability… Keywords abound, but the ideas are precise. Sarah Amrani, Clément Gakuba, Thierry Dart, Eric Théa, and Florence Tagger agree: for AI to be adopted, it must be understood, controlled, and secure.System interoperability emerges as a fundamental issue: no AI without data, and no data without collaboration. AI must also be accompanied by training, both for healthcare professionals and patients, and include real human governance. The goal is not to replace, but to strengthen human action.<h4>What we learn from the field</h4>The conference ended with the exploration of concrete cases: Jennifer Gangnard, Franck Guillerot, and Malo Louvigné shared their experiences, from virtual reality to medical knowledge transfer. Oncology meets digital health:  The Paris-Saclay Cancer Cluster and Future4Care join forces to accelerate innovation

Yet, we have only tapped into a fraction of its potential.In France, 30% of hospital doctors' time is still absorbed by administrative tasks. Too much bureaucracy, too much time wasted, and too many outdated tools. Meanwhile, AI is already capable of easing up the burden on healthcare workers, thanks to startups like Medvir for diagnostics, Hopia for anticipating hospital flows, or Tandem Health, which allows caregivers to save up to two hours per day by drafting their medical reports.For patients, the advancements are spectacular. In silico clinical trials drastically reduce time and costs of drug development. DeepLife and WhiteLab Genomics accelerate the search for new therapies using AI and digital twins.For the more than 9.3% of caregivers in France, often forgotten, AI is also a revolution. Doctoome, for instance, simplifies navigation in the healthcare journey.France has everything it takes to be a global leader in digital health. We have the talent, researchers, startups, and industry. We have an emerging Tech Bio sector, which was recently strengthened by the partnership between Future4care, France Biotech, and France Deeptech.Yet, some barriers still need to be removed for the healthcare sector to fully harness the power of artificial intelligence.<h4 style="text-align: start">Access to health data: a matter of sovereignty and competitiveness</h4>Data is the fuel of AI in healthcare. However, in our country, it takes an average of nine months from the submission of a request to access the data. Today, French startups must train on American or Asian databases because they do not have access to French data. This is absolute nonsense: It slows down innovation and prevents us from creating solutions tailored to our public health needs. It jeopardizes our health sovereignty: in the event of restrictions on foreign databases, we would become dependent on non-European players. It reduces the attractiveness of our healthcare ecosystem: startups and investors are reluctant to establish themselves in France if data is not accessible.Emmanuel Macron’s announcement of massive investments in artificial intelligence and computing power during the Artificial Intelligence Action Summit marks a decisive turning point and demonstrates the national will to advance in the race for France's digital sovereignty. By equipping the country with high-density computing centers capable of hosting and processing massive volumes of data, this strategy will reduce our dependence on foreign infrastructures and ensure better protection of sensitive information. Beyond security, these investments will accelerate innovation in healthcare by facilitating the development of AI algorithms for diagnostics, research, and personalized medicine. In the current geopolitical context, mastering health data is a strategic lever, allowing France and Europe to take the lead in AI in life sciences.<h4 style="text-align: start">Modernizing hospital digital infrastructures</h4>Integrating AI solutions into hospitals remains a major challenge. Between the budget constraints of healthcare institutions, the complexity of regulations, change management, and the process changes brought about by digital transformation, the adoption of new technologies is often slowed down. The processes for approval and clinical validation are long and costly, complicating market entry for young companies. Furthermore, hospital infrastructures are often outdated and poorly suited for integrating advanced digital solutions. Lastly, the protection of health data, subject to strict requirements, adds an additional layer of complexity.As a result, it is still very difficult to deploy AI on a large scale in hospitals. A healthcare worker who has to juggle five different software systems will never be able to fully benefit from the advantages of new technologies.For AI in healthcare to fulfill its promises, it is essential to make it easier for startups to collaborate with healthcare institutions by streamlining collaborations and financially supporting the adoption of these innovations. AI cannot function without a solid foundation. The majority of French hospitals still operate with poor connectivity and outdated IT systems, with significant interoperability challenges. We must make the modernization of hospital digital infrastructures a national priority.<h4 style="text-align: start">Access to funding: Stay the course</h4>France is home to a rapidly growing HealthTech ecosystem, supported by massive public investments and increasing private sector engagement. The recent announcement of a €1.2 billion fundraising by the French investment firm Sofinova Partners is a promising new development for the sector. However, a major challenge persists: funding for startups after their Series A round. While the early stages of seed funding receive strong support, the transition to industrialization and international expansion remains a critical phase for startups, hindered by a lack of dedicated capital. It is imperative to strengthen the synergy between public and private funding to support this crucial transition. French health startups are not lacking in innovation or ambition, but they need investors ready to take over and transform their technological advancements into industrial and commercial success. Investors have a unique opportunity to participate in the consolidation of a strategic sector that offers both growth potential and societal impact. Now more than ever, it is necessary to accelerate the structuring of specialized funds for digital health and deep tech.It’s time for Europe to take responsibility and develop an ambitious funding strategy for healthtech and AI in healthcare.Inaction has a cost. A human cost, a financial cost, a strategic cost. We have the solutions, the talent, and the startups. It’s time to break down the barriers. Learning Expedition: a morning of inspiration dedicated to responsibility and ethics

In January, Future4care launched the January4data campaign to highlight the essential role played by healthcare data in the value chain. To this end, we solicited the expertise of members of our ecosystem, which has given rise to these notes. The genesis of healthcare data, technological developments and uses, ethics and applications in the hospital sector are just some of the topics covered in this series of articles. &nbsp;&nbsp;Properly harnessed healthcare data, combined with rigorous and ethical artificial intelligence, have the capacity to implement precision medicine; a paradigm where care is tailored to each individual characteristics.&nbsp;With Scienta Lab, we aim to bring about the same revolution in the way we develop drugs, particularly for autoimmune diseases. These diseases, which affect around 10% of the world's population, are still poorly understood and difficult to treat.&nbsp;A patient-centered approach&nbsp;Autoimmune diseases such as lupus, rheumatoid arthritis and Crohn's disease are unique in their presentation in each patient.&nbsp;These chronic, incurable diseases have a major impact on the lives of their sufferers, with major, irreversible destruction of the affected organs, leading to disabling symptoms, increased risk of cancer and higher mortality.Unfortunately, current treatments are only marginally effective on a significant proportion of patients.&nbsp;Why? Because drugs are developed using a standardized approach that ignores the biological diversity of individuals.&nbsp;At Scienta Lab, we have chosen to tackle this challenge and enable the development of precision drugs for autoimmune diseases. Using data as a guide. We use health data from cells, animal models or patients to train our EVA foundation model, dedicated to immunology and inflammation.&nbsp;In this way, EVA acquires a representation of the immune system and its disorders, which is leveraged for personalized drug development. This precision medicine approach is at the heart of my vision of modern, equitable healthcare.&nbsp;Harnessing data to speed up decision-making&nbsp;In the field of artificial intelligence, the main difficulty is data accessibility. This is even more important in the healthcare sector. At Scienta Lab, we have developed an AI model specific to these constraints, and capable of extracting relevant information from small healthcare datasets.&nbsp;By studying targeted datasets and building adapted algorithms, we can answer complex questions such as:<ul><li>Will a drug work in humans, and why does it work in some patients but not in others?</li><li>Which drug candidate has the highest probability of passing clinical trials?</li><li>How can clinical trials be optimized to maximize efficacy and minimize costs?</li></ul>&nbsp;Partnerships to guarantee data quality&nbsp;An essential element of our success lies in our strategic partnerships with international hospitals and research centers. These collaborations give us access to high-quality healthcare data and ensure that our models reflect the diversity of patients with autoimmune diseases.&nbsp;We also invest in regular audits and validations to ensure the robustness of our algorithms and guarantee the security of personal data.&nbsp;Data ethics as a priority&nbsp;Europe, with initiatives such as the RGPD or the AI Act, enforces high standards on data protection and its exploitation in algorithms. These regulations impose significant constraints on innovation, but I believe they provide a necessary ethical framework to ensure that healthcare data is used for the benefit of all. Future4care Inside: A testimony from Didier Cadic, Head of Innovation at Generali France

In January, Future4care launched the January4data campaign to highlight the essential role played by healthcare data in the value chain. To this end, we solicited the expertise of members of our ecosystem, which has given rise to these notes. The genesis of healthcare data, technological developments and uses, ethics and applications in the hospital sector are just some of the topics covered in this series of articles. &nbsp;&nbsp;I'm convinced that hospitals in France play a crucial role, built around three fundamental pillars: care, teaching and research. It's a complex mission, and the rapid evolution of technology, particularly in the field of artificial intelligence (AI), is profoundly transforming these activities. Today, we are witnessing a veritable explosion of data and technological capabilities, creating immense possibilities but also raising questions of interoperability, quality and security.&nbsp;The integration of data into hospital structures is nothing new. For over fifteen years, biobanks and associated databases have played a major role in medical research. However, we now understand that simply accumulating biological data, without enriching it with contextual information (imaging, therapeutic strategies, pathological trends), is insufficient. The richness of data lies in its quality, granularity and interconnection. Clearly, without precise structuring and a coordinated effort to improve their quality, these data cannot fully meet the needs of modern healthcare.&nbsp;Unprecedented technological evolution&nbsp;Advances in computing and storage capacities, notably with the emergence of quantum computing, are revolutionizing the way we approach data processing. This technological evolution makes it possible to process massive volumes of information with greater precision and speed. For example, modern AI models can analyze data in seconds that would take months to process manually. But what does computing power matter if the data processed is incomplete or of poor quality? This question illustrates one of the main challenges we face: data interoperability and relevance.&nbsp;Take medical imaging, for example. The widespread use of MRIs and scanners, combined with ever more powerful capture tools, has democratized access to massive volumes of data. However, without a rigorous labeling process, an MRI image can only be partially exploited. It is the human expert who must, for example, identify and delimit a tumor to enable the AI to train accurately. This crucial step, known as labeling, is often underestimated, but is the foundation of any reliable analysis.&nbsp;Increased storage capabilities also enable a complete patient history to be maintained, including genomic data, medical history and interactions with different treatments. This paves the way for unprecedented personalization of care, but only if this data can be exploited and interconnected between different healthcare systems.&nbsp;The challenges of data quality and security&nbsp;Beyond simple acquisition, there are challenges linked to data management and processing. Biases in acquisition mechanisms, artifacts and extrapolation problems can all affect data quality. Take a concrete example: data from the same patient may vary according to the machines used or the protocols applied, introducing differences likely to bias analysis results.&nbsp;What's more, interoperability between different hospital systems remains a major challenge. For example, the same patient being treated in several different facilities can generate data that is scattered and difficult to correlate. These shortcomings hamper a global and coherent vision of the care provided.&nbsp;Another major challenge is data security. In a society oscillating between trust and mistrust, it is vital to strike a balance. Too many restrictions hinder access to data, even for noble objectives such as research or public policy. I've been confronted with this complexity myself: the delays and constraints imposed by systems such as CASD (Centre d'accès sécurisé aux données = Secured data access center) are often a deterrent. Yet these restrictions do not always prevent data leaks, often caused by trivial human error.&nbsp;Furthermore, examples of medical data leaks remind us of the importance of re-evaluating our systems. Some recent leaks have highlighted flaws in internal access management, underlining the need for stronger auditing and accountability protocols.&nbsp;The role of ethics in healthcare data management&nbsp;Ethics play a central role in our approach to health data. It guides us when the law is silent. But too much control can have the opposite effect, fueling conspiracy and holding back projects. We need to ask ourselves: is it ethical not to fully exploit available technologies to improve public health?&nbsp;The ethical framework is also crucial for regulating the use of AI in analysis processes. Take the case of machine learning algorithms: how can we ensure that they do not introduce systemic biases? Ethics must also address the dilemmas linked to the balance between innovation and personal data protection.&nbsp;ConclusionData is at the heart of the evolution of our healthcare system. If we are to harness its full potential, we need to rebalance our approaches between openness and protection, while ensuring that strong ethics are maintained. It's time to meet the organizational, technological and ethical challenges to build a future where healthcare is accessible, innovative and secure.<img src="https://f4c-strapi.s3.gra.io.cloud.ovh.net/image_18_6adc797844.png" alt="image (18).png" width="1204" height="1953">

Expert opinion Julien Rutard, Vice President, Healthcare Sector (Capgemini Invent) - Data in the hospital environment

To this end, we solicited the expertise of members of our ecosystem, which has given rise to these notes. The genesis of healthcare data, technological developments and uses, ethics and applications in the hospital sector are just some of the topics covered in this series of articles.&nbsp;What is your perspective on the use of healthcare data?&nbsp;For over five decades, I've witnessed and played an active role in major developments in the field of healthcare data. My work has always been driven by the conviction that quality data are essential for decision-making and research in general, and particularly for research into rare diseases, a field that Orphanet has been instrumental in helping to develop.&nbsp;When I began my career in the 1970s, the landscape of data collection and organization was sparse and unstructured, limited to research survey data due to the lack of accessible technology for digitizing real-life data. Moreover, there was no standardization of nomenclatures, which was a major obstacle to collaborative research.&nbsp;The emergence of medical informatics in the 1980s enabled progress to be made. Medical records are coded using the ICD, the WHO's international classification of diseases. The ICD listed around a hundred diseases in the XIXᵉ century, several thousand today. However, genetic diseases and rare non-genetic diseases were largely absent from this classification. &nbsp;The ICD 11 currently in use contains only 450 of these, and Orphanet has helped to introduce another 4,500 into the ICD 12 adopted by the WHO, but unfortunately not yet in use, as changing versions is costly for public players.&nbsp;At the same time, in 1966, Victor McKusick proposed the Mendelian Inheritance in Man (OMIM) catalog of human genes and genetic diseases, a nomenclature widely used in research, but which does not enable diseases to be properly coded, as some diseases are caused by multiple genes, and some genes can be the cause of several diseases.&nbsp;Drawing on my long experience in genetic epidemiology and medical genetics, I founded Orphanet in 1997, a relational, exhaustive and polyhierarchical database that considers different perspectives and reconciles the various existing nomenclatures. Today, Orphanet lists more than 11,000 rare diseases and offers classifications adapted to the specific needs of research, care, public health and the pharmaceutical industry. Orpha codes enable diseases to be coded either zoomed out (broad categories useful for health system management) or zoomed in (molecular level essential for biomedical R&amp;D).&nbsp;What are the challenges facing the use of healthcare data?&nbsp;It took a long time for medical informatics to be used, due to a lack of awareness among doctors and decision-makers, but above all because of the cumbersome nature of the equipment, the cost of the technologies and the lack of user-friendly access to the data.&nbsp;The data collections that were built up presented problems of data quality and interoperability. The databases were disparate and poorly structured, limiting their usefulness. At the same time, the constraints imposed by national and European regulations, although justified to protect nominative personal data, have complicated the use of data for research, even though they are always pseudonymized. Is it justified to protect such data to the extreme, when the risk of re-identification is extremely low, and the potential damage caused low? This position, prevalent in Europe but taken to extremes in France, slows down research, or even hinders it, even though these are desirable projects for public health, improved care and innovation. This is detrimental to clinical research in general, and industrial clinical research in particular.&nbsp;An emblematic example concerns genetic data, often considered as indirectly identifiable, which prevents their systematic digitization in medical files, even though this is key information for both care and research. Doctors circumvent these restrictions by scanning the paper results they need for care, an inefficient practice that compromises both the usefulness and security of the data.&nbsp;How do you see the future of healthcare data?Today, we have the technological tools and infrastructures to collect and analyze quality data, thanks to the construction of healthcare warehouses and the existence of cohorts, registers and research databases. However, this potential remains under-exploited due to a lack of regulatory harmonization, sufficient investment, operator training and psychological resistance. Primary health data must be protected, but pseudonymized secondary data must be reasonably released. There is no known experience of collective or individual harm resulting from the use of pseudonymized data. On the other hand, the benefits of using such data can be significant for the community (e.g. relevance of care, prevention of health scandals, evaluation of innovations) and for the individual (e.g. development of new treatments), as well as having an impact on the individuals themselves. The benefit/risk balance is therefore very much in favor of the benefits. What remains to be done is to remove the obstacles to moving towards a one-stop shop. We need to stop communicating only about risks and not benefits, even though well-informed citizens are in favor of using their data for research. An audit of the obstacles to research in the current system would be useful to convince the reluctant.&nbsp;The European health data space represents a major opportunity for us in France, as we have already structured the recommended national node. This is the Health Data Hub.&nbsp;Together, we can build a future where data is used to save lives and improve public health. On September 20, 2024, ThIA Santé Mental held expert roundtables at Future4care, in the presence of Generali teams, to discuss innovation in mental health. Psychologists, psychiatrists, researchers, representatives from the French Ministry of Health and the Caisse Nationale de l'Assurance Maladie, as well as clinic groups, manufacturers, insurers, financiers and investment funds, came together to share their vision and combine their efforts.&nbsp; &nbsp;Their objective? How can digital technology improve women's health?

From lack of access to data, to being the last link in the chain, through fierce competition, there are many pitfalls for insurers in the healthcare environment.Published on February 24, 2022 at 09:11 amThese players, in association with the social security system, play a central role in patients' lives. In the digital age, they are more than ever determined on offering personalized support tailored to individual needs, as is already the case in many other sectors. The problem is that there's a big gap between will and reality. Once again, the lack of interoperability.To discuss this hot topic, we interviewed Yanick Philippon, Director of Group Insurance at Generali France.Care pathways: organization and playersStrictly speaking, there is no such thing as a universal care pathway between patient and insurer. However, we can build on what currently exists via the Generali France customer area and its corollary, the mobile application. Today, the patient interacts with his or her complementary health insurance provider prior to treatment, for example to submit a quote, a request for reimbursement or to seek a second medical opinion before undergoing treatment or an operation. Then, once the treatment has been carried out, the patient receives the reimbursements to which he or she is entitled as a policyholder.Between these two phases, however, the insurer has no information about the patient's care path. This is the prerogative of the Caisse Primaire d'Assurance Maladie (local sickness insurance fund, "CPAM"). This information is then transmitted to the Dossier Médical Partagé (shared medical file, "DMP"), a digital health record that stores and secures the patient's health information: treatments received over the last 12 months, medical history (pathologies, allergies), test results, hospitalization reports, etc. The only people who have access to this data are the attending physician and all healthcare professionals (except occupational medicine), including hospitals.Since January 2022, existing DMPs have been migrated to a new platform called "Mon Espace Santé" (My Health Space), where new files will also be created. Until March 2022, all Social Security subscribers will be notified and will be able (if they so wish) to refuse the activation or creation of their space."Mon Espace Santé" has two main functions. First, to become one's own medical file. This will enable you to store medical documents added by you or your healthcare professionals. It will also allow you to add documents and information that predate the creation of this space, in order to retrace your medical history. In short, the paper health record we've treasured for years is going digital.The second feature is instant messaging. As patients, we can exchange or receive information or documents from our healthcare professionals.Reassurance and trust for better supportThe constraints faced by players such as Generali, through its complementary healthcare offering, are not a new issue. Patient medical data is completely sealed off from institutional players. As the CPAM states on its public information website: "Neither the occupational health service, nor mutual insurance companies, nor banks, nor your employer can access your Dossier Médical Partagé (shared medical file). Finally, any access to your DMP by an unauthorized healthcare professional would constitute an offence punishable by imprisonment and a fine".This is a constraint considering these players could better support their policyholders if they had access to some of this information. Knowing a patient's pathology, for example, makes it easier to provide specific advice on pricing without compromising the quality of care and treatment.For some treatments, variations in the fees charged can be of a magnitude that is not always justified. The same applies to prescription drugs. Wider access would make it possible to offer more precise, better-adapted health cover for the patient.Since January 1, 2022, with the opening of "Mon Espace Santé" (My Health Space), every person affiliated to the general Social Security scheme has been able to grant or restrict access to their DMP. However, this only applies to healthcare professionals. For example, a chiropodist will not be able to access your hospital reports.Another obstacle is the patient's reluctance to share his health data with his insurer. While this is absolutely not the case on the part of the insurer, there is nevertheless a fear of being selected and somehow penalized by more expensive rates, or even of being cancelled or refused complementary health cover. There is undoubtedly also confusion with the loan insurance process, where each person must fill in a health questionnaire on their honour, and may have to undergo some medical tests.Clearly, we must continue to reassure patients that there is no intention to discriminate.Likewise, there is an ongoing need for in-depth educational work to show the real added value for the patient of sharing certain data. In the case of the second medical opinion to which a patient is entitled through his or her complementary health insurance, having more data would enable a more refined diagnosis or medical advice to be given for the benefit of the patient. The more data is available to understand the patient's care pathway, the more efficient the services offered will be.Even more so, it's the ease with which data can be made available and used that is essential if patients are to benefit from the best possible prevention, the best possible care pathways when medical intervention is required, and the best possible post-intervention follow-up. In this respect, a digital health innovation ecosystem such as Future4care represents a tremendous opportunity to accelerate the development and marketing of e-health solutions based precisely on data, to the benefit of all, in particular patients and healthcare professionals.The divide between players has a direct impact on patientsThere is no denying that strong reluctance exists and persists at various levels of the value chain. For healthcare professionals, extending access to health data is very often seen as a form of increased control over their medical acts by non-legitimate third parties, or even a stronger possibility of questioning and contesting their diagnoses. Here, also, there is a need for explanation and dialogue in order to gradually overcome resistance and apprehension, and move towards mutual cooperation in the service and benefit of the patient.L’Etat lui-même n’est pas toujours exempt de certaines frictions. Le rapport du Haut Conseil pour l’avenir de l’assurance The State itself is not always exempt from certain frictions. The report by the Haut Conseil pour l'avenir de l'assurance maladie (HCAAM) on the "Grande Sécu" project (commissioned by the French Ministry of Health) is a prime example. At the moment, four scenarios for linking basic compulsory health cover to supplementary health cover are being studied. From the most radical to the most "balanced", these 4 hypotheses would de facto lead to the nationalization of health coverage and the marginalization of supplementary health insurance.If this project were to see the light of day, it would undoubtedly create a profoundly unfair system, creating a divide between those who can afford to pay more without public support and gain rapid access to the best professionals, and those who cannot.Such a scenario does not correspond to the expectations of healthcare professionals, and above all to the expectations of the French in general, who are deeply attached to a certain freedom of activity and choice.There are still things that can be done to improve health coverage for the population, in particular the most vulnerable, but this "Grande Sécu" scenario is clearly not the right decision to take. Common sense must prevail on this issue.Digital health care, the key assetThe subject of the "Grande Sécu" is divisive, while the benefits of digital technology are increasingly unifying the thinking of our ecosystem. In fact, it's difficult to be totally exhaustive about the qualities of digital technology, given the number of positive contributions it can make to a patient's healthcare pathway at any given stage. Connected devices, for example, are a formidable lever for optimized, personalized daily monitoring of various chronic diseases such as cardiac arrhythmia, asthma and diabetes, as well as for post-operative monitoring on an outpatient basis and/or at home, rather than in hospital for longer periods.Prevention is another fundamental area where immense progress can be made thanks to digital technology, for a better understanding of the patient's lifestyle and environment.For healthcare professionals (as well as for governments and insurers), predictive medicine also opens up a vast field of opportunities, such as better anticipating and curbing the onset of certain pandemics or more familiar epidemics like influenza, gastroenteritis and seasonal allergies. With the launch of messenger RNA vaccines against Covid-19, for example, the world has become aware of the fantastic progress made by digital RNA sequencing and synthesis technologies. This technology also opens up great prospects for information storage, with capacities never achieved before .The challenge of telemedicineIt's impossible to talk about digital technology without mentioning the long-standing subject of telemedicine. To be functional, telemedicine requires sufficiently powerful infrastructures and telecommunications networks. 20 years ago, access to fiber optics and mobile broadband was still in its infancy in terms of deployment across France.Today, fiber and 5G have a much higher coverage rate, although there are still white zones where patients and healthcare professionals cannot use telemedicine. As a result, the use of telemedicine will be facilitated and amplified.Furthermore, beyond the question of infrastructure, we need to encourage the use of remote healthcare services as soon as possible. In this respect, a very interesting step forward was taken on January 27. The French National Authority for Health (HAS) published four guidelines to help healthcare professionals make the most of remote medical monitoring. These guides define the conditions for prescribing telemonitoring, the minimum technical specifications required for digital medical devices and collection accessories, the conditions of distribution and use, the professionals involved, the professional qualifications required, the organization of telemonitoring, the minimum provisions necessary to ensure quality of care, and the follow-up and use of telemonitoring.By July 1, reimbursement for remote medical monitoring will be generalized for several pathologies (diabetes, renal failure, heart failure and chronic respiratory disease). In the near future, patients with implantable cardiac prostheses will also be covered. For healthcare professionals, this means they can remotely interpret data for their patient's medical follow-up, and even adapt their treatment. This remote monitoring can detect a worsening in a patient's state of health, enabling them to intervene more rapidly. These devices are real assets in demonstrating the immense benefits of digital technology in healthcare.Future4care, catalyst and gas pedal of progress in digital healthcareFuture4care has a fundamental role to play in this digital health ecosystem. In both cases, digital technology plays a central role, transforming our entire industry. This quest is that of e-health sovereignty. E-health will play an increasingly central role in the provision of healthcare, as well as in the organization of the healthcare system. The supply, organization and development of treatments must be thought out on a European scale, otherwise we will not be able to establish our own sovereignty.This is vital, not only for the production of goods, but also for the creation of value in the short, medium and long term. It's a matter of reconquest, since we have too quickly left this subject in the hands of other geographical plates. Digital technology is still in its infancy, and could well be our best ally in this reconquest.We need to move swiftly from aspiration to operation, and organize on a European scale with a clear, easily deployable roadmap.Health data as the driving force behind e-sovereigntyIn the West, as well as in the East, the ideology is relatively different from that of Europe. The European Union, the world's third-largest economic power, is still struggling to position itself as the instigator and federator of European e-health. Notably because there are still major disparities between member states.Digital technology creates value and is an opportunity for economic growth, job creation and attracting investors and talent.Faced with the temptation to turn to solutions originating from North America or China, Europe must give itself the means to create real conditions for the deployment and massive use of healthcare data, by research teams, start-ups and industrialists, and resolve the ecosystem's challenges when it comes to using this data: facilitating the identification of sources and pooling them, providing for European data storage, establishing interoperability, avoiding the multiplicity of national legislations, guaranteeing the valorization of this data by regulatory and assessment authorities, ...Data protection is another interesting example. Europe was a forerunner with the RGPD, and showed citizens that protecting their data was important. We therefore have a secure framework to develop innovation on health data and health solutions. China is in the process of strengthening its legislation, which allows the state to access all data generated on its territory, under the banner of national security, and without any guarantee of protection of either personal data or intellectual property. Europe can therefore assert itself as an attractive and competitive region in comparison to the USA.To achieve this, European institutions need to harmonize and pool their approaches. In other words, they must fight against a heterogeneous vision among its 27 members, and avoid repeating the same mistakes made in the past decades with regards to medicines. Europe has a multitude of approaches specific to each country, making it difficult, until now, to produce and distribute on the same scale,In 2022, if we want to be attractive and successful, we need to help our European start-ups avoid an uphill battle for evaluation and market access, so that they can develop rapidly in our market, then expand and move to the USA or elsewhere. Because the reality today is that some of our most ambitious entrepreneurs think first of the market across the Atlantic, even before establishing themselves in Europe. Fund-raising is easier and faster in the US because the market is unified, and the launch of a healthcare solution takes place de facto in a vast, harmonized market. We need to guarantee them a secure, interoperable and harmonized space.&nbsp;In the short term, the European Union must take three steps in particular:<ul><li>Propose harmonized regulations throughout Europe to guarantee simplified, secure deployment of healthcare data, which is the area of innovation par excellence for start-ups.</li><li>Guarantee European markets to encourage the emergence and industrialization of our future champions, first on our own territory, before exporting them.</li><li>Interconnect the dozens of European data initiatives, such as the Health Data Hub in France. These initiatives, however brilliant, will only be of limited interest in the medium term if they are not interoperable. At least if we wish to guarantee a market large enough and solid enough to compete with the East and West.</li></ul>&nbsp;Evaluate to guarantee reliable industrialization over the long termObviously, going fast and offering a clear, predictable framework is only a short-term advantage. In e-health, more than anywhere else, this is not a sprint, but a marathon. A key challenge in creating value in e-health is evaluation. It is essential to guarantee safety and efficacy, whether through CE marking for medical devices or, of course, clinical trials for drugs. We still need to find the right evaluation model for digital health products and services.This logic must obviously remain demanding for the good of patients, without becoming counter-productive, and it must be thought out on a European scale. Once an e-health start-up has validated its technology, it should be able to deploy it rapidly, rather than knocking on 27 assessment agency doors. Research and development work is already risky, so we need to make the evaluation stages as easy as possible, and support our entrepreneurs before they go to market.This does not mean reimbursing all devices and solutions, but rather formalizing and harmonizing approaches and methodologies, for example to measure the impact of solutions that generate efficiency gains in healthcare. In short, we need to move towards more co-construction, and put real resources in front of entrepreneurs...Co-investing in risk with entrepreneursDigital reconquest in Europe's healthcare sector is also a question of multidisciplinarity. The relationship between the private and public sectors, on a local scale and even more so on a European scale, is perceived as a power struggle linked to structural and cultural differences. Yet they are interdependent. The role of public authorities is to ensure that the framework in which we evolve is appropriate. At the same time, private players – whether they are manufacturers, investors or entrepreneurs - play an initiating role. The former need solutions that respond to the problems encountered by citizens, while the latter need favorable regulations and the capital to design and deploy them.Today, however, it's difficult to obtain financing when there's a risk involved (regulatory, but not only). That is what's holding back the rise of our start-up ecosystem. It is in their nature to take risks, and we need them to innovate. The same applies to manufacturers.&nbsp;&nbsp;There is a whole acculturation process to be thought through around this notion.Today, we have a logic centered around subsidies (research tax credits, etc.), but no logic of co-investment in national and European risk. In the USA, the crisis has highlighted the role of the BARDA agency, which has invested massively in vaccine research, but which existed long before the crisis and whose role is to co-finance risk with private companies, whether in R&amp;D or industrial deployment. The co-financing of risk is one of the strong points that will enable Europe to find its identity and achieve health sovereignty. We have been able to do this in the field of defense, in a regal logic, since the public authorities do not question the vitality of defense. The creation of the future HERA agency, and its industrial arm Eu-FAB, is a step in the right direction, but we need to speed up its creation and operationalization, so that projects by start-ups and larger companies can be boosted and anchored in Europe.It's all about risk. The bigger the risk, the bigger the companies resulting from it wil be.A clear framework for risk-taking is what will set us apart from other geographies, and attract talents capable of thinking up solutions. They need to have confidence that risk-taking is part of our culture.&nbsp;Banking on multidisciplinarityBeyond taking risks, consolidating a unified vision of healthcare data will undoubtedly be key to regaining a central geography. Of course, not all European Union countries are committed in the same way or at the same pace. Moving forward together means accepting that we have to deal with each other's differences. With its Presidency of the EU in the first half of 2022, France is in a position to make progress in the field of health data.<ul><li>Defining the rules of the game and interoperability. In other words, creating a secure, easily accessible and value-creating structure.</li><li>Including the citizen. Helping them understand that the challenge is not to track data, but to create value for them, for their health, for progress, and so on. And to make them contributors to this new system.</li><li>Helping patient associations. These key players have understood, particularly in the case of rare diseases, that registers and monitoring are needed to save time and win the race against time in the face of these diseases.&nbsp;</li><li>Reducing inequalities. Sharing data will enable us to rationalize our healthcare system and help every citizen benefit from a personalized, and therefore fairer, system.</li><li>Preventing future crises. Data will guarantee the resilience of the healthcare system, thanks in particular to its predictive promises.</li></ul>What's certain is that we can't achieve these goals without multidisciplinarity and risk-taking. What's more, healthcare professionals are there to support and advise them. And it is all the easier when digital tools are at their service to simplify this support.That said, there is still an important issue to be addressed: making e-health inclusive.Indeed, the state of health tends to deteriorate, particularly when advancing in age, which is a risk factor, especially for chronic diseases. And it's this more vulnerable target group that we need to focus our efforts on if we are to help.It is essential to step up training, adoption and dissemination to these more vulnerable populations of the tools they need to manage their own health. And it's the job of healthcare professionals to democratize these devices, to explain, inform and train patients themselves in the use of these useful tools.&nbsp;N°2 <a target="_blank" rel="" href="https://youtu.be/N2TVUbOlyU0">Healthcare professionals</a>, also, need to take the plunge and embrace digital technologyOf course, there's no such thing as a healthcare system without professionals. In the current system, the attending physician is a keystone, since he or she is the integrator of all health data for the patient.However, as time goes by and public health needs change, we realize that it is a succession of players in the healthcare system who are effective for the patient.Thus, the players in the healthcare system in the broadest sense are obviously the professionals who must act, but who must act in a coordinated way.What better way to coordinate than, in particular, with digital solutions and products that enable this very specific exercise of patient care and its coordination.However, digital tools by doctors, and GPs in particular, are currently regarded as gadgets. How come?Because the French healthcare system was built on a succession of therapeutic acts. That is why we pay for consultations.But public health needs have changed, notably with the chronicization of illnesses, so we also need to change the way we, as healthcare professionals, see the service we provide to patients. This requires us to work differently, and to do so we need different tools, which is what digital innovation is all about.We need to be able to analyze all the data we need, or at least to have the tools that enable us to draw signals from it, so as to be constantly vigilant. This will enable us to avoid the so-called decompensations of chronic diseases, which are at the root of the deterioration in our fellow citizens' state of health, when treatment is not up to scratch. In the end, it's a great opportunity, but it also changes our vision and practices.Digital tools in the healthcare sector frighten health professionals, or generate anxiety, quite simply because they force us to work differently.However, we have chosen our professions because we want the best for our patients. This conviction is shared by all healthcare professionals.&nbsp;It's up to us, as professionals, to take up organizational issues and say to our supervisory bodies: "Ultimately, the model you're putting us into - these stories of one-off consultations paid for one after the other, for example - doesn't suit us. It doesn't suit us, because it forces us not to do our job properly. And that, for us, is unbearable."Basically, when a tool allows us to do better, we end up adopting it.&nbsp;&nbsp;N°3 Digital technology to unite <a target="_blank" rel="" href="https://youtu.be/oLc211kc53U">private and public players</a>The healthcare ecosystem is quite complex, since it includes not only healthcare professionals, but also - and this is what Future4care is all about - manufacturers capable of bringing us new solutions. And let's not forget other players, too often in the shadows, such as local authorities. In particular, town halls, departments and regions. These are key players when it comes to economic development and, in particular, prevention. There is also a French specificity. Patient associations. They are essential players to include in this ecosystem, because they have the extremely detailed knowledge we need. Generating innovation without them is virtually impossible.&nbsp;Having said that, it is well known that there's a long-standing French opposition between these private and public players.&nbsp;The presence of a professor and doctor as Vice-President of Future4care is not insignificant. It's also to make the link between these two types of players, whether in healthcare or industry.&nbsp;&nbsp;The aim of this rapprochement is, quite simply, to be of service to patients. As soon as this objective is shared, there is no longer any boundary between the private and the public sectors, as long as the necessary safeguards are put in place, in terms of respect for the patient, respect for patient confidentiality and respect for the patient's person and autonomy. We all need to be on board.That's how we will live up to the expectations of our fellow citizens and ourselves, as potential future patients.However, it is clear that there is fragmentation, due to this ancestral opposition. This fragmentation in the healthcare system, which is so frightening, is internal to what we traditionally call healthcare. But it is also accompanied by two other axes. As the WHO says, health is medical, psychological and social. The health axis, as we understand it, is medical. But there is also psychology. We are experiencing this with Covid, we have experienced it with mass tragedies, like the 2015 attacks on our territory. Finally, the social axis. Health is inseparable from our social environment and our integration into society, especially for the most precarious and vulnerable among us. It's for them that we need to make the most connections, and for whom digital tools are also the most useful.This fragmentation is therefore manifold. Let's take up the challenge together, in a coordinated way, to prove our good will. Digital tools will help us achieve this, because they are the best levers for defragmentation of the healthcare system, provided that their genesis, this co-design of innovation, is already shared by patients, by healthcare professionals, with the manufacturers who bring us the solutions.&nbsp;&nbsp;What role does Future4care play?Patients are at the heart of the system. There can be no innovation without co-designing it with them, because they will be using these innovations for their own health. This co-construction is Future4care's DNA. It's only once everyone agrees and comes together that we'll be able to implement these new solutions, which are first and foremost intended for patients and all citizens who want to preserve their health throughout their lives. Harnessing the wealth of healthcare data will revolutionize the way we care for patients. It's the basis of the "5Ps" of digital medicine: more personalized, preventive, predictive, participative and evidence-based.&nbsp;Rather than argue about the potential, which you've all already clearly identified, the idea here is to understand how to move from intuition to action. To do this, we spoke to a specialist in the subject.&nbsp;Laurent Frigara, co-founder of Enovacom, the French leader in medical interoperability and an early supporter of Future4care's development.In charge of Orange Business Services' healthcare division, Enovacom currently employs over 250 healthcare experts.Here, he talks to us about sharing culture, interoperability and methodology.&nbsp;A little backgroundToday's healthcare professionals expect the right data at the right time, for the right patient. This is the foundation on which the entire medical decision chain is built. Whether it's about test results, vital signs from biomedical devices and sensors, prescription histories or health episodes, sharing this information is essential. This sharing ensures better patient care and improved care coordination, while avoiding the redundancy of examinations already carried out.&nbsp;&nbsp;The challenge is also to exploit health data in a consolidated way and in very large quantities. For example, using data from large cohorts of patients with common characteristics opens up interesting prospects and promising advances for healthcare and medical research. We can detect the emergence of certain diseases at an early stage, speed up diagnosis and evaluate the effects of medical treatment with greater precision.Data is also the raw material for AI learning. The quality and performance of an AI is directly linked to the quantity and quality of the data used.&nbsp;On a facility scale, the effective use of data from its IS enables it to optimize its operations, and thus improve care by adapting processes based on concrete elements.&nbsp;Proportionally, data consolidation is more important at regional level. It enables us to adapt health policy or, as we saw during the health crisis, fight effectively against a pandemic.&nbsp;At the same time, there's no denying that health data is also coveted for less virtuous purposes. They are so precise and personal that, for example, they can easily be used for identity theft. It can be traded at a premium on the dark web, or used to ransom healthcare facilities.&nbsp;With this in mind, we'd like to share with you five essential lessons for the successful digital transformation of your company, institution or other healthcare structure, with an effective, reliable data strategy at its heart, at the service of healthcare.&nbsp;N°1 Dealing with market constraintsMore than anywhere else, the healthcare sector faces multiple constraints, whether from an organizational, regulatory or economic point of view. Today's healthcare establishments, which produce and manage vast quantities of data, have inadequate information systems, and their transformation is slow in coming. Most of them are made up of several application layers from different generations, ten, fifteen or even twenty years ago. Under these conditions, data sharing is much more difficult.&nbsp;Historically, French doctors have rarely been inclined to share patient information. This culture of sensitive, anonymous and exclusive data locks usable data into information silos that are not conducive to exchange and sharing.&nbsp;&nbsp;What's more, it's the patient who owns his or her data. When they give their consent for their personal information to be used, they do so within a certain framework, which can sometimes hinder the use or consolidation of existing databases.&nbsp;&nbsp;To remedy this, healthcare players need to work on information systems, to make them interoperable and standardized, based on common frames of reference. To optimize the use of healthcare data, we need to ensure that it is properly collected, structured and of sufficient quality. It is also necessary to adapt processes that integrate patients consent, so as to obtain a clear vision of where the data comes from and how it will be used.&nbsp;&nbsp;N°2 Learning to structure and communicateWe need to create optimal conditions for these exchanges, both in terms of security, trust, and the ability to share reference systems to better understand each other. It is therefore necessary to put in place a technical foundation that is standardized, common, understood, integrated and shared by all players. Enovacom has been working in this field for 20 years, and it is also one of the missions of the French National Health Agency, which, through financial aid from the Ségur de la Santé, is accelerating this harmonization. This deliberate drive by the French government is an opportunity to ensure that the information systems of all healthcare organizations are based on the same standards.&nbsp;The digital solutions of the various players need to communicate with each other, if not natively, through dedicated complementary services. This will also enable them to exploit the structured data contained in healthcare documents, to offer users even greater relevance.&nbsp;However, incumbent publishers in the ecosystem do not make their often proprietary databases easily accessible. Interoperability services, whether on site or in the cloud, can break down these silos by positioning themselves in an agnostic way.&nbsp;When it comes to upgrading their information systems, healthcare establishments need to ensure that future healthcare devices, applications and software are interoperable by their very nature, and that the data they generate can be exploited. In other words, the healthcare data generated by these new elements of the healthcare ecosystem must be stored in structured data warehouses, based on common repositories. Everyone needs to speak the same language if they are to make the most of the data they collect. &nbsp;&nbsp;N°3 Be methodicalUnfortunately, knowing how to juggle with market constraints and initiating an open approach are not enough. A great deal of care needs to be taken at every stage of the data journey to prevent it from losing its value. If just one stage is not carried out correctly, the data is no longer qualified and therefore unusable. Meticulous care is therefore required, from inputting data into care production software, or sampling vital data from biomedical equipment, right through to storage in a suitable database.&nbsp;This means providing caregivers with ergonomic software natively designed to store data that can be used. Tool training is also essential to ensure the quality of the information entered.&nbsp;In addition to standardizing data syntax for sharing and consolidation, it is also important to work on semantics. In the first instance, this concerns the definition of the message: its nature, type and format, so that it can be understood by all.Semantics concern the medical content of this data, and require the sharing of semantic interoperability repositories. &nbsp;The Systematized Nomenclature of Medicine - Clinical Terms (SNOMED CT) is a key terminology for meeting these challenges. It is an international descriptive terminology covering the widest range of clinical specialties and operational needs. It standardizes the capture, classification and sharing of any clinical information by healthcare professionals.Data quality control tools also exist, to ensure that the data exchanged complies with these different standards and repositories. One example is Enovacom's work on the SI-DEP project, where its solutions analyze and control the structure and semantics of data transmitted by analysis laboratories.&nbsp;&nbsp;N°4 Generalize interoperabilityWe can't talk about digital health without addressing the issue of interoperability. Interoperability refers to the ability of different digital systems, applications and services to communicate and interact with each other. In particular, it enables patient information to be shared throughout the patient’s journey. Interoperability also facilitates the integration of innovative solutions or services into an existing establishment or ecosystem. Ultimately, this helps secure the care process, simplify procedures for patients, improve the relevance of medical decisions and ensure more effective prevention, etc.The many digital solutions used by healthcare professionals need to be interoperable, not only with each other, but also with government services such as the Personal Medical File, the Digital Health Space, regional platforms, and data warehouses such as Health Data Hub.Interoperability is also a key factor in the exploitation of healthcare data. It enables data to be extracted or consolidated from existing systems to feed data warehouses, and repositories to be shared to ensure that everyone speaks the same language.&nbsp;&nbsp;N°5 Data at the heart of Future4care's business modelIt's important for players in the ecosystem to be supported by specialists in collecting, storing and using healthcare data.&nbsp;&nbsp;Future4care's ambition is to support start-ups in bringing their innovative solutions to the market faster.&nbsp;&nbsp;In addition to providing exclusive, qualified data, Future4care supports European start-ups throughout the lifecycle of their solutions. We enable them to focus on their core added value or differentiator, as they often have little time to break through and competition is fierce. This highlight will take place during France Digital Day, France's largest business meeting place for startups, financiers and manufacturers.From the Musée des Art Forains in Paris, we'll have the pleasure of launching this first call for projects, focusing on two themes: personalized medicine and remote medicine. This will be followed by a meeting session with a selection of e-health entrepreneurs from the France Digital network.&nbsp;What is France Digital?France Digital is an association of 1,800 French and European digital entrepreneurs and investors. Its board is made up of well-known investors and entrepreneurs such as Benoist Grossmann of the Eurazeo fund, Frédéric Mazzella, founder of Blablacar, Jean-David Chamboredon of the ISAI fund, Fleur Pellerin, Axel Dauchez and many others. The organization also runs France Digital Day, one of Europe's most exclusive start-up events. Each edition brings together some 3,000 start-up executives, industry c-levels, venture capitalists, opinion leaders and decision-makers from the global tech scene, from over 40 nationalities, as well as 400 VCs.&nbsp;How to apply for the Future4Care call for projects?To join the first class of our e-health start-up gas pedal, you need to meet the following four criteria. Firstly, your company must be at an intermediate level of development, close to "go to market". Secondly, your business must fall within one of the two themes on which the first promotion of our gas pedal is focusing. These are personalized medicine and remote patient care. Please note that your company's head office must be based in Europe. Finally, you must be able to justify that you are devoting 100% of your time to this project, and that you intend to make it your priority for the next few years.
